Physicians can schedule surgeries in advance by completing the International Cornea Project Tissue Request Form.  Prior to the date of the surgery, an ICP  coordinator will contact you to help select a cornea for the surgery.  The selected cornea is then packaged in wet ice and shipped to you, and all shipping information is emailed or faxed to your office.

 

To schedule a surgery in advance, please print and complete the International Cornea Project Tissue Request Form and fax it to 858-694-0581 or email the form to icp@sdeb.org and provide the following information:

 

· Patient's name, sex, age, race, date of birth, address, and 

      indentification number.

· Patient's diagnosis, operating

      eye, and requested tissue

· Surgery Date & Time

· Surgeon's Name, Contact          Person, Telephone Number &

      Fax Number

· Surgery Site, Telephone

      Number & Address

· Purchase Order Number
